Enrolment Information

To download a Southside Health Enrolment Pack click on the button below. Alternatively you can ask one of our reception team to email you or pop in to the clinic and pick up an enrolment pack.

Each person enrolling must complete an enrolment pack and all patients 16 and over must sign their own form.

All new patients will be required to book a longer ‘New Patient’ appointment at the first visit which is charged as a one off set fee. This visit includes a free 15 minute nurse appointment which is used to collect and update information to help streamline your appointment with the doctor.

There is currently a 2-3 week wait for new patient appointments. We suggest ensuring you have adequate medication from your previous practice before completing your new enrolment with us. If you require urgent medical attention and are not already registered with a GP in the area we will do our best to fit you in for an appointment sooner.
