Becoming a Registered Patient
We are currently enrolling new patients for all our GP’s – you are welcome to call in to have a look around our surgery and pick up an Enrolment Pack – to download the Enrolment Pack click here
Please note the enrolment pack requires a PDF reader, if the document does not open please ensure you have one installed – you can download Adobe Reader for free here
All persons aged 16 and over are required to sign their own enrolment form.
Completed packs must be either posted or hand delivered back to our surgery. They cannot be faxed or emailed as original signed copies must be kept on file. Once your forms are received Southside Health will then request your medical records from your previous doctor.
